1095-C Forms:

As required by the Internal Revenue Service (IRS), your Form 1095-C documenting employer-provided health insurance coverage is now available. You may access your 1095-C electronically through the Paychex Flex online portal. To do so, log in to your Paychex Flex account at www.paychex.com and navigate to the "Tax Documents" section, where your 1095-C will be available for viewing and download.

If you prefer to receive a paper copy by mail, please contact the corporate office at (972) 263-2112 or email klowe@calabinc.com. Upon request, a printed copy will be mailed to your address on file within 30 days, in accordance with IRS guidelines.
